How to prepare for an evaluation
- Bring a current medication and allergy list.
- Share relevant medical notes, imaging reports, and prior test results.
- Describe previous medications, therapy, injections, procedures, or surgery and how you responded.
- Tell the clinician about anticoagulants, infections, allergies, pregnancy, or other health considerations when applicable.
- Identify the activity or functional goal you hope to improve.
Do not stop or change a prescribed medication unless the prescribing clinician instructs you to do so.
